Enabling Aging in Place
Enabling Aging in Place will bring together health and social service organizations from across the healthcare continuum to implement promising practices that are delaying entry to long-term care and improving safety, health and quality of life outcomes, while also reducing emergency department visits, lessening demands on care partners, and making better use of health and social care resources.
How the collaborative works
The collaborative will be delivered in two phases.
During Phase 1, teams will receive support to create an implementation and evaluation plan that helps them develop a sustainable aging in place program that builds on the strengths of individual communities and responds to the local needs of older adults and their care partners.
Phase 2 is expected to run from June 2024 to September 2025. Qualifying teams from Phase 1 will be supported to implement, spread or expand a promising practice based on their implementation and evaluation plan.

Healthcare Excellence Canada offers a range of supports to collaborative participants, including:
Phase 1
- Seed funding of up to $15,000 per team
- Virtual and in-person networking, coaching and learning events to promote sharing and collaborations across Canada
- Access to evidence, resources and other capability building supports
- Measurement and evaluation support
- Opportunities to guide the development of new resources to support aging in place programs as well as community-specific needs
Phase 2
- Opportunity for up to $25,000 in seed funding per team to support implementation activities as well as a continuation of all capacity building, learning and networking supports offered in Phase 1.
- Virtual and in-person networking, coaching and learning events to promote sharing and collaborations across Canada
- Access to evidence, resources and other capability building supports
- Measurement and evaluation support
- Opportunities to guide the development of new resources to support aging in place programs as well as community-specific needs
How to apply
Applications to Enabling Aging in Place collaborative are due by November 15, 2023. Please submit your application form by email to EAIP-FVCS@hec-esc.ca in English or French.

HEC will notify you of the outcome of your application in December 2023.
Who should apply
Organizations that support older adults with health and social needs to age in place are encouraged to apply, including:
- Primary care providers, health teams and clinics
- Community-based organizations
- Long-term care and continuing care organizations
- Social or private housing
- Acute care facilities
- Municipal, provincial and territorial governments
- Regional health authorities
- Indigenous governments/organizations
Applicants must be developing, spreading or expanding an aging in place program that prioritizes older adults living in the community who have complex medical needs and are more heavily impacted by structural and social determinants of health.
We welcome groups at various stages of their quality improvement journey. However, applicants must already have a team in place that can develop and implement an aging in place program, two years of funding to support this work, and a commitment from senior leadership.
Applicants must be prepared to sign a collaboration agreement by January 5, 2024 to participate in the program. Review the sample collaboration agreement for more information.
