[Survey] OurCare National Survey 2025

In this survey, we will ask you questions about your experiences with primary care, and about you. You can also choose to fill out the survey on behalf of an adult you care for, sharing their experiences and who they are. We will use the results to advocate for changes to the system that will make care better for everyone.

What’s involved?

The research survey takes about 15 minutes to complete and will be available until 23:59 ET on 9 July 2025. You must be 18 years or older to participate in this survey.

• Your participation in this study is voluntary.
• You can stop and return to your survey at any time by clicking on the link below, using the same web browser. If you stop and do not complete the survey, your answers will not be used.
• Your responses are de-identified. We don’t ask for your name or other directly identifying information in the survey. This also means that if you complete the survey and change your mind about participating, you will not be able to withdraw your data.
