[Article] Cochrane launches social prescribing pilot for older adults
The Town of Cochrane is launching a two-year social prescribing pilot designed to help older adults connect with community services that support their health and well-being.
The program, funded by Healthy Aging Alberta, provides a dedicated social prescribing link worker who will work with local healthcare providers and older adults to connect them with non-clinical supports in the community.
The initiative supports Action 7 of Cochrane’s Age-Friendly Action Plan 2025-2029, which calls for improved connections to healthcare resources through education and system navigation.
Last November, the town was recognized as an Alberta Age-Friendly Community, joining a growing network of Alberta municipalities committed to creating inclusive and accessible communities for residents of all ages. Cochrane became the 13th Alberta community to receive the designation.
