[Workbook] Supporting Albertans to plan ahead with the My Wishes Alberta workbook

My Wishes Alberta is a workbook to help Albertans think about what is most important in their life, health and personal care, and to share this information with those who matter most, and with their health care providers. While the My Wishes Alberta workbook does not replace formal advance care planning documents such as a personal directive or Goals of Care Designation order, this workbook can help people prepare for important care decisions now and in the future.


Whether you're a health or social services professional, member of a community organization, caregiver, or volunteer, the My Wishes Alberta workbook can support your interactions with Albertans about how to plan for the future. This free resource helps Albertans start advance care planning by encouraging them to think and talk about what matters most to them.

  • Making sure your wishes are honoured, with links to additional resources for next steps


The workbook is available in a fillable electronic format (regular and large print versions) and in a non-fillable format for printing and completing by hand (regular and large print versions). Professional print copies are also available to order. Learn more here.


My Wishes Alberta was adapted from Coming Full Circle, a resource developed by the Canadian Virtual Hospice and a national Circle of Elders and Knowledge Carriers.
